急性软组织扭伤是指四肢关节或躯体部的软组织(如肌肉、肌腱、韧带、血管等)损伤，不包括无骨折、脱臼、皮肉破损等情况。
临床主要表现为损伤部位疼痛肿胀和关节活动受限，多发于腰、踝、膝、肩、腕、肘、髋等部位，大部分由剧烈运动、负重持重时姿势不当或不慎跌仆、牵拉和过度扭转等原因引起某一部位的皮肉筋脉受损，导致经络不通，经气运行受阻，瘀血壅滞局部而成。
临床常碰到患者一扭伤后就马上涂抹“红花油”“活络油”“驱风油”或对患侧进行搓揉按压甚至敲打等处理，此类行为均不可取！
踝关节在急性扭伤时，为什么会马上肿胀？
这是因为其关节周围软组织受到关节的外力作用，挤压撕裂等引起关节周围毛细血管破裂出血导致局部肿胀、疼痛，而此时再用活血药物进行搓揉会加重毛细血管的出血，从而加重了肿胀情况。
急性软组织扭伤的中医特色疗法
第一步
立即停止行走、运动等
在这个病程期间都应尽可能制动，佩戴护具或者是用绷带、三角巾等布料加压包扎踝关节周围。亦可用数条宽胶布从足底向踝关节及足背部粘贴、固定踝关节，减少活动度。
若是外侧受伤，应使足部呈外翻的状态；如果是内侧受伤，则应使足部呈内翻的状态，减少对肌肉或伤处的牵拉，从而减轻或避免加重损伤。
第二步
立即冷敷，减少局部出血及肿胀程度
在伤后局部肿胀尚未消失的24-72小时内，每1-2小时冰敷10-20分钟，在冰块和患者皮肤之间最好垫上薄毛巾或者是衣物。冰袋应紧贴受伤的部位。
第三步
可用弹力绷带加压包扎，减轻水肿
在伤后24-36小时佩戴，但绷带仅用来消除水肿，对于关节并没有保护作用，所以若需要踝关节承重则需要支具支撑。
第四步
经前面处理后，第三天即可涂抹速效止痛酊
组成：三七30g，川芎30g，红花30g，栀子30g，元胡30g，当归20g，鸡血藤30g，伸筋草20g，乳香20g，没药20g，95%酒精mL。
制法：密封浸泡7天后备用。
功效：活血化瘀，消肿止痛。
适应证：软组织损伤，骨折及关节脱位早期；爱运动者练功前后使用，可防劳损。
用法：将速效止痛酊适量外擦患处，亦可配合按摩手法，有伤口者不能用。
第五步
恢复期后做功能锻炼，防止再次发生
有过一次损伤的患者尽量穿高帮鞋保护踝关节，不做剧烈运动。
实践证明，中药速效止痛酊是治疗急性软组织扭伤和颈肩腰腿痛的理想方法。
其优点是局部用药，药物直入病灶，病灶周围药物深度高。急性软组织损伤疼痛是由于局部气血瘀滞而引起，正所谓“通者不痛，痛者不通”，而本药具有通经活络、祛瘀止痛之功能，因此本疗法可达到药到病除之目的。
另外本药使用方便，无毒副作用，值得在所有基层卫生室推广使用。
